End-of-Year Student Reflection & Feedback with Google Forms

  Of all the Google tools, Google Forms might just be my favorite.

It has revolutionized the way I use exit slips, schedule writing conferences, give quizzes, assign book clubs, collect information — the list could go on.

Since it's that time of year for us teachers and our students to get our reflections on, I created a Google Form to help make that process a bit smoother.

In the download, I included two versions of the form, though the questions are the same on both. Version 1 is the one you see on the left side of the photo with the orange text and prettier fonts. The old version of Google Forms has themes that were much more customization than they are now, so sometimes I like to revert back to the old version for the font change alone. Version 2 is the one you see on the right side, with the plain ol' standard black font. If you're new to Google Forms, I recommend Version 2 as the editing aspects are more user friendly.

I used this form with my 8th graders last year and was blown away by the thoughtfulness of the responses. It asks questions like, "What did you find most challenging this year?" and "What could your teacher have done to make your experience in this class better?" which encourages students to reflect on their year, but also provides me with important information to help me reflect on my year as well. I didn't require students to write their names, but many of them chose to anyway.

And because it's a Google Form, submissions are automatically stored in a Google Sheet — which means I can view the responses any time from any device. Forever and ever.
